Mental Health Connector
Living Well Consortium is recruiting in collaboration with Birmingham Mind and Better Pathways for a Mental Health Connector. They will work within the Neighbourhood Mental Health Team (NMHT) which provides integrated care through GP surgeries across Birmingham and Solihull. The post holder will offer support for both mental and physical health, addressing issues like money, housing, relationships, trauma, addiction, and more.
The post holder will identify and address barriers such as social anxiety, financial concerns, or childcare issues, creating tailored plans with Service Users. This can include offering practical support, like social stories or travel plans, as well as attending initial visits to new venues. They will deliver a short-term (up to 6 sessions) person-centred interventions using a holistic approach.
Successful candidates will have 12 months' paid or unpaid experience supporting individuals with their mental health.
This role has been created in collaboration with Birmingham Mind and Better Pathways.
This role is designed to cover Primary Care Networks in East Birmingham. Support is provided in clinics, communities, remotely, or at Service Users' homes.
Terms and Conditions

- Working hours: Part time, 35 hours per week
- Salary: £26,264.89 pro rata per annum
- Fixed term to 31st March 2027
