IAPT Long Term Conditions
Living Well Consortium (LWC) are proud to secure the delivery of the IAPT LTC service across Birmingham and Solihull.
LWC is a member lead consortium with a strong history of delivering the IAPT contract for BSol CCG, including young people’s services for FTB and IAPT triage service for Worcester Healthy Minds.
From the 1 December 2019 our service to support those with anxiety and depression arising from Long Term Conditions & Medically Unexplained symptoms will go live.
Starting the programme
To kickstart the programme we will cover practices in areas of Erdington, East Ward, Kings Norton, West Birmingham and Solihull.
Our aim for the next 2 years is to cover all PCN’s for Birmingham and Solihull and to ensure all relative users have appropriate access to the IAPT LTC service.
Interventions
We will deliver a range of interventions that are tailored to the needs of the client.
- CBT (Cognitive Behaviour Therapy)
- Health & Wellbeing programmes including self-guided support
- Wellbeing groups or coping groups
- Information and support in living with LTC and MUS
Waiting times
Our waiting time are 2 to 4 weeks.
Our clinically trained IAPT practitioners (Talking Therapists) and Health Psychologists, located within or very close to the GP’s surgeries will deliver this service. All therapist are fully qualified with the LTC top training from Sheffield University.
The referrals will be generated by working with GP’s and through the MDT pathway. We are already part of the OMNIA MDT in the East and this has been an effective way of working to identify the needs of service users for the most appropriate therapy.
We accept self-referrals, this will be done via an assessment to ensure the service users are appropriate for the LTC pathway, confirming details with the GP before an intervention is proposed.

The conditions we will be supporting through LTC pathways are;
- Diabetes
- Chronic obstructive pulmonary disease
- (COPD)
- Cardiovascular disease (CHD)
- Musculoskeletal problems (Chronic pain).
- Chronic Kidney Disease
- Irritable bowel syndrome
- Chronic Fatigue Syndrome
- MUS not otherwise specified
Our service delivery model will have the standard baseline of the Integrated IAPT treatment pathway with an active social prescribing strand.
